John Blacet sells his Input module that would do the trick. 1/8" jacks, though, which can be tricky with guitar even if you have a nice STooge 1/4 to 1/8 cable. The torque from walking around and stuff could mangle the not-so-robust jacks. An alternative is to build that into a Stooge Panel or a Schaeffer panel.
