Paul Maddox's midi2sds2 can do it with a small mod to the output. But if 
you're not a tech wiz the MTM is easier.

and, as response to the Q's sent: MTM is 2U high, rackmounted and no 
longer in production. The second hand prices are in the region of 
100-250 euros imho.
