There are several iterations of the Z-SYS Digital Detanglers with various configurations of AES, S/PDIF and Toslink/ ADAT i/o to patch with.
The specific one I'd been looking for is the Z-SYS Z-8.8A Lightpipe Digital Detangler.

Thanks for the lead; in recent years (trying to go digital audio) I have wondered if anyone made such a device--but could never find anything based on the key words I was using.
