I've had the same problem myself (on a Mac). I think you're 
right--the exs just loads the first instrument with the right name it 
finds in the list of instruments. The only thing I've found to do is 
delete or rename duplicate instruments so that every instrument 
has a unique name. The only disadvantage with this would be if 
you've used both versions of the instrument in different songs. 
The renamed one would come up as the unrenamed one.

This moves the raw source file on disk only. The archive index is not changed automatically, so you still need to run a manual refresh afterward.
