The new site, from Larry's friend Doug, is www.stoogepanels.com, but I'm not sure what's going on with it. Maybe if you email him he will make you a couple. I heard he's been doing them informally like that, but that was a year or two ago. 

The Miniwave is in the Bridechamber queue, but a ways down. I think most folks who wanted one already have one. 

Another option is to see if there is a  .fpd file somewhere, and order from Schaeffer/ FrontPanelExpress.

I built a non-VC version with the Hylander expansion board that uses a
thumb wheel to select the ROM. I really like the thumb wheel. You can
use my FPD panel if you like, although it's my own quirky design.

I think my panel FPD file has two toggle switches. (FPD is on my other
computer, so I can't check right now.) I retrofitted the thumb wheel
when I added the Hylander expansion board. (I sure do remember drilling
and filing those rectangular holes! Yes, I did two of them.) So, if
you want to use a thumb wheel, I highly recommend doing the rectangular
hole in Front Panel Express! BTW, you don't need the side extension
pieces to the thumb wheel, that Dave used. Here's Dave's page:
