Tom G collaborated with someone on the Synth DIY list to make a Pitch to CV
circuit board. He may still have some left. You can contact him at:

One that was produced commercially and supposedly worked well was the Gentle
Electric. I think Dave Fulton used to have one, so he may be able to
comment on it.

you're actually looking for the Korg MS03, a stand alone pitch to CV &
envelope follower
i think i have a PDF of the user manual, but there's plenty of stuff on the
web about it, just dial in:

if you're looking for pitch to CV, look up the Roland SPV-555, it's
allegedly quite nifty
