I had this problem with all of my SDS9 pads at one time or another after a few months of use, and I think Jesper is right, or close to it. I found in each case after taking off the bottoms that one of the wires from the piezo was just barely disconnected from the contact, and then when the pad was hit hard enough, the wire would flap around a few times, make intermittent connection, and cause a double or triple hit.
