Not trying to point you out as a moron, but are you triggering from the
midi keyboard and sending the trig to _just_ ring or _just_ tip to check?

If so, and it works from the button on the machine, I'd say it's a
simple loose wire between socket and the prom circuit. Or did I miss
anything here? Loose wire can also be a bad soldering OC but in general
it sounds as if the external control to that specific prom circuit is
the villain...
