Another good source for a quality on/off switch in a box would be the 
sustain pedal for any MIDI synth.  At least with the  Ensoniqs they were 
routed so that a single stereo 1/4" plug was wired for power to one of the 
terminals, ground to the other, and the output of the switch to the 
third.   Wiring up the adapter to this would require minimal soldering.
