On my white-face, both oscillators behave the same, ie., neither of
them "disappear" when the ADSR PW mod is selected.  If it's just Osc 2
that is the problem, I suspect that it's because of the 50% adjustment
you set.  Think about it: if you set a thin pulse to start and then
modulation it with a non-cyclical wave, it might go to 0%.  It's
possible that it doesn't seem to disappear with the LFO modulation on
because the LFO is cyclical and keeps reappearing.

Try re-setting the 50% adjustment for Osc 2, and also check to see
that the LFO/ADSR select switch isn't dirty.  It may be that the
process of replacing the nearby slider may have knocked a bit of dirt
into the switch.

I've got three sliders with broken stems that I should replace on mine.
