You'll need one MUUB-4 PC Board, available here for $10.

The panel has been designed and is expected to be available as a Stooge 
panel.

You'll need a few extra pots and jacks and a bracket to remount the 
board.  You know, I think I will put up the parts list on my site.  I 
have all the parts and the boards are assembled, waiting for the panel.
