how do I remove single entries from the database?

You can stop miler-greylist, and remove the line from the database file
(it is text format).

You can also set up an ACL to do it, using the flushaddr action.

If you have a sync address, is it OK to telnet to the port to do the 
delete? Also, how expensive is it to do mass deletes? I have 148 thousand 
entries this morning. If I deleted 10k entries in a minute would that be bad?

Sure. One day someone will write a CLI tool to explore and change the
database using this way.

Everything is stored in RAM, it should be fast.

Move to quarantaine

This moves the raw source file on disk only. The archive index is not changed automatically, so you still need to run a manual refresh afterward.
