This was caused by some DNSRBL checks taking up to 30 seconds.

I pushed up the R timeout value in sendmail.cf (ex: "Xgreylist, 
S=local:/path/to/socket, T=R:1m") and disabled the faulty DNSRBL in 
greylist.conf, and all seems to be fine now.

Sendmail's default R value is quite low (5 seconds), one may have to 
pump it up to something more comfortable (30s or more) for each 
milter-greylist setup involving DNS-intensive tasks such as SPF, DNSWL 
our urlcheck; Maybe a word about this in the installation doc may be 
helpful ?

By the way, when in "verbose" mode, milter-greylist 4.0rc1 logs response 
times for SPF checks, but not DNSRBL and urlcheck queries. The two 
attached patches corrects this issue. Very handy to diagnose those 
timeout issues...
