I'll agree that you might want to provide some way of isolating your input for the protection of this IC. It's hard to tell from the photo, since the max resolution I can view it is 400x400. I think you have it correct. If the end of the IC with the semicircular "notch" is up, then the first pin to its left is pin 1, then the pins are numbered sequentially counterclockwise.
