Burst Generator issues

My freshly assembled Burst Generator is sending a continuous stream of
clock pulses on the pulse out and the Duration out is always high. In
other words, the counter doesn't seem to be working.
This problem is reminiscent of Richard Brewster's issue that he solved
by using a different 40106 make:

However, I already have a TI 40106... but my 4017 is a Fairchild.
Should I swap it out?
I tried swapping the lm358 for a variety of opamps and the only time
this behaviour changed was when I used an OPA275, in which case the
pulses would only send when holding down the trigger button (although
the duration was still continuously high). Weird!
Any thoughts? I checked for all the seemingly obvious things such as
correct parts and solder bridges. No dice.
