It has 2 disadvantages : the drilling should be made manually and the image doesn't have exactly the same dimensions as designed - this way the automated drilling it's not possible directly. For Mach3 driven, cnc owners I have made a solution based on a webcam mounted aside from drilling motor, a modified video plugin and a small program for conversion/rotation/scale of excellon drilling files.
