I've been searching for this as well. The practical advantage of doing a single board with all knobs, jacks and switches on the same surface is massive just in terms of reduced wiring, imo. The best I've come across is this [1] style - which is switched (nice) but has the ground connector off the side (ouch). It may be the same as, or similar to, toby electonics UJ-3506. See also Lumberg 1502 03 [2], and Kycon ST-3002-3NM. I'd be interested to hear of other solutions.
