You've been running MIS rather than OEM, so if a clog it's a "famous 3rd-party clog", assuming it is a clog. If not a clog, of your MIS carts may have lost a chip or otherwise damaged the contacts (quality control issue). Look around inside the cart slots for junk or damage.
