Hi Richard: Apart from inkjet, not much will match 175-200 linescreen printing using duotone or quadtone inks on a high end press. There's also http://asukabook.com/ but costs are considerably higher per book compared with blurb. I believe all are printed on an offset press, but not with duotone/quad inks. For short run, the quality of Blurb's black and white can be excellent. The cost is not high to run a test, which is the best way to know whether the quality is acceptable to you. The more I think about it, if you make prints that you like the look of, and a paper you like, it's not a bad idea to print the pages on inkjet and have a custom bookbinder do the rest. Of course, there are a number of reasons why someone might not want to go in this direction- time and cost come to mind.
