DTXPressII TP65 Tom Drum Pad Falls Off Drum Pad Clamp

HI Gang. I was playing the other night and my tom (right front) drum pad fell off its mounting. I have it tilted down from cross-bar horizontal position on an approx. 45-degree angle when I'm facing it. Apparently, drum pad (TP65) slides off the rod that fits inside of it. I didn't over tighten, etc., but it appears that I can't tighten it anymore. Anyone run into this before? Any suggestions? Thanx!
