I don't think it's a bad 3340, since you said you'd swapped other parts  
into this position and they acted the same way. The obvious next  
question is "Does the chip from the bad position cal up ok when  
inserted in another voice?" If so, that pretty much rules out a fault  
in the 3340. Hope so, for your sake - 3340s aren't going to be cheap.
Assuming it's ok, you're looking for dead components around the 3340 in  
the bad position. It could be a dry joint, or perhaps a dead capacitor  
- they'd be more likely to die of old age than a resistor.
