Sounds like  a voice getting out of tune.

Might be a bad trimmer, but more likely a VCO chip dying.

I recommend to add a LED to each voicecard, with which you can detect, which voice is used: http://www.florian-anwander.de/yamaha_cs60/
Touching the trimmers for the tuning is another method, but it is quite tiring.

I am sure you will finde the culprit soon.
