Mu-tron bi-phase jack wiring

Hello everyone,
I
am restoring an old mu-tron bi phase. The previous owner had
internally disconnected the footswitch jack to have it function without
the foot switch. I'd like to put it back the way it was... except I
don't have the wiring diagram and the schematic doesn't have every
connection listed. does someone have a wiring diagram for the bi-phase
foot switch? or even a picture of the foot switch jack from the back?
I would much appreciate it!
I have looked all over the internet for a picture of that din jack from the rear, but to no avail
