Typically this isn't caused by a hardware issue, probably all sounds are 
generated on a single IC, if something goes wrong it just all dies at once 
rather than losing polyphony.

It's likely either a poorly-chosen setting (multitimbral allocation of 
voices?  accidental layering? a midi cascade function for using multiple 
modules together?) or just a software malfunction (reset it).
