Did you have forced formating after the "iF"-message?
The "iF" can also show a other format an the disk - if it was not blank
(for example the "empty" disk is IBM pre-formatted etc). 
Press then Execute again and the Sampler should format the disk. 
(Save->Fd->Exec->perhaps u'll get the "iF" message -> Exec -> P2k 
shoud format now!)

Move to quarantaine

This moves the raw source file on disk only. The archive index is not changed automatically, so you still need to run a manual refresh afterward.
