You could try my JTAG interface design in jtag.zip. I know that works OK as 
I use it myself. The one you are using won't work with OCD Commander without 
a slight mod. - pins 8 and 15 on the DB-25 connector need to be connected. 
The current OCD Commander software looks for that connection.
