RME Hammerfall DSP series is excellent. The PCI card version and the
Cardbus version both work with  Multiface which has:
8 in/8 out Analog
Midi in /out
Adat in/out
Spdif in/out
Wordclock 
etc

There are other versions (Digiface, new HDSP model) if you want more analog
via ADAT. 

This setup runs at low latency with VSTi's and Logic's audioinstruments and
is absolutely rocksolid on PC. Reports for the Mac are generally very good
from what I read but I have yet to use mine with a Mac. The company has an
excellent forum for info on hardware issues etc and the chief developer
answers questions there promptly. This is what I would buy again if I lost
mine.

Delta 1010 is cheaper and is also good but not as efficient with CPU.
It has:
8 in/8 out Analog
Midi in /out
Spdif in/out
Wordclock 
Does not have ADAT I/O but you can get an extra box from M-Audio which
allows ADAT connection to the Delta1010.

Others report good results with MOTU products on the Mac but no so great
with PC.
