You'll have to build one up yourself. You can buy any old 26-pin ribbon
cable with the standard two-row connectors of the appropriate length, such
as:

Then you can chop off one connector and replace it with:

which is probably compatible with the four-row pattern on the panel board.
Make sure you chop off the correct end; the connector is keyed so it can
only go in one way.

I don't have any guaranteed ideas on how to do the mass termination, but
embedding the connector pins into a chunk of rubber (a large eraser
perhaps), and then squeezing the whole thing in a vise would be my first
guess. I also can't guarantee that these are exactly the right parts, but
they look right to me, after a quick glance at the parts drawings.

Note also that the some Polaris cables had a ground plane, while this one
does not, but it isn't really necessary.

They also make a 3' version of the cable, if you want some extra service
length.
