Master Divider finished - is it functioning correctly?

I'm not sure if my divider is working as it should. There are 3 issues..
on /2 phase 1&2 light/trigger at the same time, and phases 3&4 go at the same time.
/8 goes from bottom to top. (lights go from phase 4, 3, 2, 1, where the others go 1,2,3,4)
There are always 2 LED's lit per division channel. 1-2, 2-3, 3-4, 4-1, cycle
Otherwise, it seems to work. If I plug 4 drum modules into any division (other than /2) they trigger sequentially. Top to bottom on /4 and /16 and bottom to top on /8.
