A couple of clues:
1) You only need to do the modifications specific to your version of the
board. Ver 0.2 modifications do not need doing.
2) Decoupling capacitors are to keep the power rails stable. These parts are
already on the PCB. You do not need to do anything extra.
I don't have any V0.4 left, so hopefully someone else can answer the questions.

board. I have version 0.4 of the PC board. The first mod in the REV. 0.4 is

not put in the jumper or should I go ahead and do it since this is a later
version.
with -15, 0, +15. Does that mean I don't have to do any modifications to the
