I've been working with someone else on this problem. If I could
find the 14-pin Centronics cable headers, I'd build DCB cables
for folks, but I can't even locate these.

A different solution may be to replace the 14-pin DCB with a
standard 15-pin D sub connector and just wire up the 14
pins. It would be a custom job for your individual pieces of
gear, but at least it would work.
