OK... not sure this is it, although it certainly points in the right direction. I've posted it to my blog:

But in a nut-shell, CS4 and 10.6.2 have issues with version 4 ICC profiles.

Move to quarantaine

This moves the raw source file on disk only. The archive index is not changed automatically, so you still need to run a manual refresh afterward.
