According to a MacTeX expert:

"Under OS X 10.11, El Capitan, writing to /usr is no longer allowed, even with Administrator privileges.”

I think QTR copies some files to a subdirectory of /usr.

This moves the raw source file on disk only. The archive index is not changed automatically, so you still need to run a manual refresh afterward.
