Select the layers you want to print before printing. Type DISPLAY <enter>. 
Or click "Display" in the toolbar.

For the top copper: turn off everything except: Top, Pads, Vias, Dimension.

For bottom copper: turn off everything except: Bottom, Pads, Vias, 
Dimension.

For top silkscreen: turn off everything except: Dimension, tPlace. After 
selecting tPlace, turn off tDocu and optionally one or both of tNames or 
tValues.

For bottom silkscreen: turn off everything except: Dimension, bPlace. After 
selecting bPlace, turn off bDocu and optionally one or both of bNames or 
bValues.

For drills, first define the drill set. On the main menu: 
Options|Set...|Drills. Select Set. Do this once for every board; it updates 
once it is created. Run the drill-legend uber-macro: from the main menu: 
File|Run... select drill-legend.ulp. This creates a new layer Drill-Legend, 
and draws the drill guide and legend. To print, turn off all layers except 
Dimension and Drill-legend. I delete the drill-legend when I'm done so the 
board extents shrink back down. This keeps the other artwork consistent in 
size.

I haven't found a satisfactory way to combine the images on one sheet. 
Photoshop is one way to do this. Try typing the command: "export image 
c:\temp\foo-TOP.png monochrome 600" to export the (currently displayed 
layers) as a monochrome PNG. Export the other views the same way.

Reverse the top copper image in Photoshop with Ctrl-I. Flip the image with 
Alt-I, E; mirror horizontal. Expand the canvas to make room for the other 
views. Alt-I, S; horizontal 200% relative to the top left.

Open the bottom artwork image. Reverse it with Ctrl-I. The image is already 
flipped so you won't have to. Copy and paste the entire image into the 
expanded top artwork. Ctrl-A, Ctrl-C, Ctrl-TAB, Ctrl-V. The image is pasted 
in the center, which is just fine. (I draw a border around the image so I 
can see the image boundaries: on the Paths tab, click convert to path; 
select stroke path with brush.)

Open the top silkscreen. Reverse it, flip it, and copy and paste into the 
expanded top image. Move it to the right edge: V, to select the move tool. 
Shift drag the image to the right edge. Use the cursor left and right keys 
to fine tune the placement. You want the marching ants to just disappear on 
the right edge, and then bring them back one pixel with the left cursor key.

Ctrl-P to print... yada yada.
