I was looking for something else the other night,  when I came across 
the guide from Kodak.

It is named "KODAK REFLECTION DENSITY GUIDE", it is a guide to be used 
with the "Kodak Graphic Arts Exposure Computer", copyright 1958. There 
is no other reference number on my card.
