Hi Les,
A couple of thing to try
 - Check hat the ISP freq is < 1/4 Xtal freq (Under 'Board' Tab the ISP freq 
combo box)\
 - in the 'advanced' tab try to read the signature bytes (does not solve 
anything but interesting..
- Try to manual erase the device (in 'Program' tab)  before trying to 
program flash (this resets the fuses as well)

An the some 'simple' ones (Ok, sorry to insult you ;-) ) but ...
- in the 'Program' tab - check that the 'Programming mode' settings are
    - 'Erase Device before programming' checkbox set
    - 'ISP' checkbox set
