Look in the back pages of Electronic Design. There are usually about 6 PCB 
houses advertising in there. I use PCB123 for layout. They also have a board 
shop. Protos tend to always be pricey but they do excellent work with fast 
turn.
I'm finishing a proto project. The boards are about 2" x 3.5", 4 layer. Four 
protos cost a tad under $75 each. No shorts or missing traces. Plated thru 
holes.
