carries a 4x4 matrix keypad like you are asking for it is a

As you can see they have 4x4 and 5x4 matrix keypads without legend. 
Legends ordering info are on this page also.

The legends I ordered were black character rub-ons and clear plastic 
film that went over each finished key.
