Hi Jeff-
Could be an SMS-3 card like mine, if it was mounted
in the late '70s. Mine has the card model number
stenciled on the bottom of the card, opposite the big
capacitor, near one corner (I believe). It is just
visible under the heatsink. Wedge a mirror behind the
corner, see if you can see it. Yes, yours has a novel
mounting location, but the heatsinks are oriented
correctly.

Interesting to note that my unit is #1221, just a
few minutes younger that the one you mentioned. My
motor controller is mounted on the wooden subchassis,
just above the power supply (where the CMC-10 once
resided).
