You have 1 bad voice. Calibration of that one voice will probably look after
things. Although it is a little strange that you have both a bad pitch and a
volume difference.

You will probably find that it occurs in POLY 2 mode as well, but you have
to hold down several notes (up to 8) to trigger the bad voice.
