Is the cap on the wiper of the crossfader a little small?  It might be a little bass-light with .001uf there.

Not that it's a design flaw, but keep in mind that it inverts the signals it's passing.  This can cause no end of confusion in some situations, but for a simple headphone amp, it's not fatal.

You might experiment with other values of crossfader, as well.  It's being loaded by the 10K resistor, and at the center either signal is something like 12 dB below where it is at the end.  You might find that it's a little touchy to adjust.  A lower value will be more immune to the loading.  Are you after any particular gain structure through this circuit? (IE: when faded to one side, the output should be +/- how much, or when centered, one side should drop by how much?)
