While assembling a power supply I hooked up the transformer backwards (
20vac winding to 120vac side ) ( not my fault, manufacturer had put the
sleeves on the toroid windings incorrectly ) *and* grabbed the wrong bridge
rectifier so there was a diode basically straight across the now approaching
1kv winding... ( totally my fault )
To top it all off the fuse I got was *NOT* UL listed and was made from some
form of flammable plastic and not ceramic so it just went up in flames and
kept conducting.
After I put the fire out and my ears stopped ringing I was able to salvage
everything past the bridge rectifier.
