Quantizer errors are inevitable.  You can always position the input
signal right
between two codes in the A/D converter... and it will dither.

You could eliminate the dithering by adding hysteresis. SO far I'm
thinking about
doing that by using a D/A, subtracting the result from the input signal,
then adding
a small amount of this error signal to the input BEFORE it hits the A/D
converter.

I just found a reference to an Analog Hysteresis circuit in the Analog
Devices Non-linear
circuits book... this uses a deadband circuit (like those used on some
pitchwheels, for example)
and a fast integrator.  One it locks a signal... it will remain there
until the input moves out of
the deadband.  Could this be used ?   I have not thought it through yet.
