[sdiy] Lost AN5296 app note!

I've lost my: 
Application Note AN5296 “Applications of the
CA3018(Ca 3046) Circuit Transistor Array”
for Suggested Applications.

Does any one have this old and
farty app note in PDF format?

Its an old RCA note sold to Harris, then
put into the trash bin by Intersil!
