  According to my ancient RCA databook, a CA3048 is a quad AC amplifier 
designed for single-ended supply (+16VDC max).  Non-inverting inputs have 
higher impedance (90K typ.) than inverting inputs (about 10K). Open-loop 
bandwidth is 300KHz.

 Amplifer:   A B  C  D
 inv inputs: 3 7 10 14
 ninv input: 4 8  9 13
 output:     1 6 11 16

 Vcc: 12 15
 GND:  2  5
