One thing to consider (but not crucial) is to recalculate
R9, the res from noninv input to gnd, it should be
calculated from parallel feedback res and all summing
input resistors - ie. you have 2 inputs (100kX2)
then r9 is 33k (10k trim is not important)

And it's summing opamp. Not sure if mixing would mean
something different here.
