I recently worked on a CS80 that had a bad level switch. I looked all 
over but could not find a matching slide switch. I ended up mounting a 
dpdt toggle switch in its place. If I were doing this again, I'd just 
hardwire the high-level outputs to the jacks and skip the switch.  In 
the meantime, you should be able to use the headphone output.

Thanks for the tip David. I was using the headphone output and after I posted this I got 
brave and just rebuilt the switch since I had all the pieces. Works great now.
