I've had custom cases made by Anvil and USA Case. Calzone also makes
custom cases. Afaik, so do Mid Atlantic and Cadence. Also, many SR and
touring companies make their own cases, and might take on outside work if
their shops aren't busy. Almost any company that sells pro gear can get
cases made for you, and might have a good idea based on previous orders who
can make the case you want at the best price.
