I think we have the same boards. If so, just use the parts list on the thread Ross linked to, and see the fix for the led later in the thread. You'll need to hijack one of the spare holes next to a cap for the +15v for the led.
There are no docs that I know of, besides what's available on the cgs site, and that e-m.com thread. If you use the web wayback machine page, you can find the old banalogue site, which has some example patches that might help you test it once it's built.
