You can get SMT jumper pins but you have to consider that they do not have the mechanical strength of a through hole part. They'll probably be fine for jumpers where they're not changed very often but probably would not be suitable for ICSP as the stress of plugging and unpluging the cable as well as pull on the cable while it's connected will probably break the joint. It might be acceptable if the ICSP is designed for firmware upgrading once or twice during the life of the product but generally through-hole would be recomended.

Digikey has a variety of SMT headers but you probably won't find too much with .100mil centers.
